Comprehensive Home Safety Assessment
Our therapists identify medication mismanagement, fall hazards, and environmental modifications to help individuals age in place safely and with greater independence.
In-Home Caregiver Training
One-on-one education sessions designed to help caregivers understand and manage challenging dementia-related behaviors such as agitation, defiance, anxiety, and personality changes.
Cognitive Evaluation & Treatment Interventions
Speech Therapy: Improve short-term memory, word finding, focus, and executive function.
Occupational Therapy: Enhance daily routines, community integration, schedule organization, and IADLs.
Physical Therapy Evaluation & Fall Risk Assessment
Our team assesses physical and cognitive fall risks and develops personalized interventions to improve strength, balance, and confidence in movement.
Virtual Monthly Caregiver Workshops
Led by healthcare professionals and dementia specialists, these supportive group sessions explore real-world caregiver challenges and offer a space for shared learning and community.
Dedicated Dementia Navigation Team
We serve as a bridge between caregivers, primary care providers, neurologists, and local resources—offering routine meetings to coordinate care and ensure ongoing support.
Psychosocial Wellness Assessment
We evaluate the emotional and social impact of dementia on both patient and caregiver, offering targeted tools and strategies to support well-being and reduce caregiver burnout.
Up to $2,500/Year in Respite Support
In partnership with the GUIDE Medicare Dementia Initiative, caregivers may be eligible for up to $2,500 annually in respite care services to ease the day-to-day caregiving burden.
